Treatment of Normal Tension Glaucoma (NTG)

Normal Tension Glaucoma (NTG) is a type of glaucoma where the intraocular pressure (IOP) is within the normal range, but the optic nerve is still damaged. NTG is a chronic condition that can lead to blindness if left untreated. The goal of treatment for NTG is to protect the optic nerve and slow or prevent further vision loss.

Medical Therapy

The first line of treatment for NTG is medical therapy. Medications such as ocular hypotensive agents, which lower the IOP, can be used to protect the optic nerve. These medications work by reducing the amount of aqueous humor produced by the eye or increasing the outflow of aqueous humor from the eye.

Another class of medication that can be used in NTG is neuroprotective agents. These medications are used to improve blood flow to the optic nerve. Some examples of neuroprotective agents are:

  • Carbonic anhydrase inhibitors
  • Prostaglandin analogs
  • Rho-kinase inhibitors

Laser Therapy

If medical therapy is not effective in controlling the IOP, laser therapy may be used. The most common laser therapy for NTG is selective laser trabeculoplasty (SLT), which uses a low-energy laser to target the trabecular meshwork, the area where aqueous humor drains out of the eye. This procedure can improve the outflow of aqueous humor and protect the optic nerve.

Surgery

If laser therapy is not effective, surgery may be recommended. The most common surgery for NTG is a procedure called non-penetrating deep sclerectomy (NPDS). This procedure creates a new drainage channel for the aqueous humor to flow out of the eye, improving blood flow to the optic nerve and protecting it.

Monitoring

NTG is a chronic condition that requires regular monitoring to ensure that the treatment plan is effective. The patient will need to have regular eye exams to check the IOP, optic nerve, and visual field. If the treatment is not effective, the treatment plan may need to be adjusted.

Prevention

While there is no known way to prevent NTG, early detection and treatment can help slow the progression of the disease. It is important to have regular eye exams, especially if you have a family history of glaucoma or other risk factors such as a history of cardiovascular disease, hypertension or age above 40.

In conclusion, Normal Tension Glaucoma (NTG) is a type of glaucoma that occurs when the pressure inside the eye is within the normal range, but the optic nerve is still damaged. The main goal of treatment is to protect the optic nerve and slow or prevent further vision loss. This may include medications, laser therapy, and surgery. Regular monitoring is essential to ensure that the treatment plan is effective and to adjust it if needed. Preventive measures include regular eye exams, especially for people with risk factors for NTG.
